Title • Forename(s) • Surname • DOB • Address • Email • Mobile telephone number • Spectacle prescription details • Contact lens prescription details • Eyewear purchases • Contact lens purchases • Direct Debit details

WHY WE CONTACT YOU

The Oculist contact you for your continuing Eye care- to let you know you are due for an eye examination or contact lens check up or other clinical event. Direct Marketing- to inform you via email news letter of any upcoming events including promotions, sales and parties.

HOW WE STORE YOUR DATA

Your data is stored on our secure practice management software and on secure simple spreadsheets and on filed record cards in secure drawers. Access to all of your data is via secure, encrypted login.

WHO HAS ACCESS TO IT?

Employees of The Oculist are able to access your data by logging into the practice management software via encrypted login.

WHO DO WE SHARE IT WITH AND HOW?

We share your personal data (your surname only) with optical suppliers when ordering products on your behalf via email or paper order which is sent via secure courier. All of our 3rd parties are also data compliant. We share your medical records with other medically qualified professionals e.g. Doctors to maintain your eye and general health.

HOW CAN YOU ACCESS IT?

You can access the personal data we hold on at any time by emailing or writing to The Oculist Opticians. We will have your record files ready within 48 business hours.
